Carvedilol 12.5mg Dosage
Heart failure: 3.125 mg by mouth twice daily with food; may double every 2 weeks as tolerated to a maximum of 25 mg twice daily (50 mg twice daily if over 85 kg).
Hypertension: 6.25 mg by mouth twice daily with food; may increase every 1-2 weeks to a maximum of 25 mg twice daily.
Post-MI left ventricular dysfunction: 6.25 mg by mouth twice daily with food; titrate every 3-10 days to a target of 25 mg twice daily.
