What Is Leucovorin 5mg?
Leucovorin is a form of folinic acid, an active version of the B vitamin folate. It is most often prescribed as a "rescue" therapy after high-dose methotrexate to reduce harm to healthy cells. It is also used when methotrexate or similar folic acid antagonists are eliminated too slowly or given in excess, and to treat megaloblastic anemia caused by folate deficiency when oral folic acid therapy is not feasible. Leucovorin tablets are taken by mouth on a schedule set by your provider.

Rescue therapy after high-dose methotrexate in osteosarcoma
Reducing toxicity from inadvertent methotrexate overdosage
Counteracting effects of impaired methotrexate elimination
Toxicity from other folic acid antagonists (e.g., trimethoprim, pyrimethamine)
Megaloblastic anemia due to folate deficiency when oral folic acid is not feasible
Leucovorin 5mg Dosage
Methotrexate rescue: 15 mg PO every 6 hours for 10 doses, starting 24 hours after the beginning of the methotrexate infusion; adjust per methotrexate levels.
Megaloblastic anemia (folate deficiency): up to 1 mg PO daily.

- Fainting or severe dizziness
Warnings & Interactions
Contraindicated in pernicious anemia or other anemias caused by vitamin B12 deficiency
Enhances the toxicity of fluorouracil (5-FU); combined use requires close monitoring
May counteract the antiseizure effect of phenobarbital, phenytoin, and primidone
Oral dosing is not appropriate when nausea, vomiting, or GI toxicity may impair absorption
Does not treat other toxicities of methotrexate, such as kidney injury from drug precipitation
Tell your provider about all medications, including trimethoprim-sulfamethoxazole
Tell your prescriber about all medications, supplements, and health conditions before starting leucovorin 5mg.
